When you arrive for your first appointment, please bring the completed Patient History Form and questionnaires we mailed to you when you made your appointment.  You may also download copies of the appropriate forms by clicking here.  This will help in developing a treatment plan to suit your individual needs.  Make sure that you have included any medical conditions or procedures that your parents or siblings may have had.

Also be sure to bring your insurance card, if applicable.

At your first appointment, your history will be reviewed and then the doctor will perform a musculoskeletal exam.  At that time she will determine if there is a need for further testing, such as x-rays.  The techniques she may use include, but are not limited to, gentle manipulation, massage, exercise, physical modalities and nutritional support.
